While you have may have filled out an Emergency Medical Authorization Form for the district, we also need one on file for the choral department. Because we have activities outside of normal school hours, we need to have an E.M.A. on file with the choir department. If they would like, your parents can simply print the one you fill out for the school before they “submit” it to the district. Otherwise, you can simply print a blank one from our choir website, fill it out, and bring it in.
